Would you like to purchase an ad in the program booklet? 1/2 page ads cost $100 and a full page ad cost $200. All ads must be purchased and submitted no later than 45 days prior to date of the conference. There is a $20 fee for program advertisement preparation.

Please select the number of tables that are needed for your display. Tables cost $200 each if checks are received before August 30th. There is a 2 table maximum. Tables cost $225 each after August 30th.

Do you need electricity? Electrical needs are handled through the hotel. This information will passed on to the sales representative for the hotel.

Are you willing to donate an item for door prizes for the Monday, October 26 Luncheon?
